2019-2020 Student Forms Packet are now available online !
Student Forms Packet are now available online and due by August 23rd.
In an effort to streamline opening of school preparations we kindly ask that you print and complete all of the forms included in the packet attached below. Complete each form fully and legibly, once completed staple the packet including the cover page and submit this packet to the students first period teacher once school begins.
Students who do not have access to print packets may request a printed copy from their 1st period teacher once school starts.
All students are required to complete and submit a packet no later than August 23rd.
